The Valley West Community Hospital Medical Records Department will be happy to provide you or your healthcare provider copies of your medical record. Your medical record contains information regarding your personal health and the care that took place at Valley West Hospital.
In order to assure confidentiality and to comply with the law, we may need you to sign an authorization for release of your health information. A copy of this authorization is available below or obtained from our office located on the first floor prior to entering the Medical Unit. A nominal fee may be charged in order to obtain your records.
The Medical Records Department is available during the following hours:

- Call the Medical Records department to notify them of your request at 815.786.3741. We can get the records ready for you ahead of time so they are ready for pickup.
- Bring the completed form with you and a photo ID when you pick up your records or fax the form ahead of time to 815.786.3743.
